Cancellation Letter (Subscription or Membership)

This Cancellation Letter can be used to cancel a subscription (such as a magazine) or membership (such as to a gym or club). A cancellation letter is basically a form of communication to inform a service provider, institution or company that the writer is dissatisfied with the membership or subscription and would like to discontinue it.

Sending a cancellation letter provides proof of your request, and decreases any chance of misunderstanding.

How to use this document?

This document allows the member to fill in the necessary details and is formatted as a formal business letter.

It will specify the membership or subscription that is being cancelled, as well as the account number. It also gives the opportunity to provide a reason for the cancellation: you do not need to provide an elaborate reason, but you may provide context for the cancellation.

This letter also asks for a receipt to confirm the membership or subscription has been cancelled. If there is a fee for cancelling the membership or subscription, it should be included in the letter. If any money is owed to you, such as a member you have cancelled before the membership period is up, it will request the money to be refunded by check.
