Instead, this document covers a broader range of circumstances, such as medical leave, family emergencies, or other personal circumstances that would require an employee to take a formal leave of absence. Though at some companies, it is acceptable to make an oral request for a leave of absence, many companies require a formal written request. Even for employers who do not require a written request, making a request in writing is typically a good idea so that there is a record of the request for future reference in case of later dispute.

This letter covers a range of different circumstances, including the use of unpaid or paid leave they have accumulated while working, the use of Family Medical Leave Act (FMLA) leave for medical emergencies, or an employee requesting a leave of absence that is not covered in by official time they have already accumulated with the employer. This contract details the scope of the employment and outlines specifics such as leave policies and related matters. An employer may also have a Vacation and Leave Policy that covers extended leaves of absence.


How to use this document
