The Sick Leave Request Letter is a letter used by an employee to take a leave of absence from work due to an illness or injury. An employee may make sick leave requests orally or in writing. However, it is better to send a formal written request, as it documents the employee's leave request. This document will form part of the Human Resources file on the employee's absences at work. It shows the reason and duration of the sick leave.

Some employers would prefer a written leave request. However, the employee must consult the Human Resources manager to ascertain the most suitable format for the sick leave request.

This document is should only be used when an employee is ill, injured, or is advised by a medical professional to take some time off from work to rest for health reasons. It can not be used to request a leave of absence or holiday, but the Leave Request Letter should be used instead. Also, please note that this document should not be used where an employee has been diagnosed with COVID-19, as the COVID-19 Sick Leave Request Letter is available for that purpose.


How to use this document

In this document, information, such as the nature of the illness and the contact details of employee requesting the leave is requires. The employee may also enclose their medical certificate or report to support this application.

After completing the document, it should be printed and signed by the employee sending this letter. The signed letter can be sent either in a letter form or via email to the employer or the required to handle leave requests in the organization such as the human resources officer, the employee's supervisor, or any other officer at the organization.


Applicable law

There is no special law regulating sick leave letters due to the coronavirus. However, the Labour Act 2004 regulates matters affecting employees, including sick leaves.
