The Childcare service also known as day care is taking care of the child by either individual or institution in the absence of the parents.

The Childcare Agreement is a document that can be used by parent(s) and guardian who are availing the childcare service or childcare service provider providing the childcare service.

This Agreement can be used by an individual providing childcare services or an institution like a crèche, kindergarten, nursery, etc. The Provider shall be above the age of 18 years with no criminal record.

This Agreement will help both parties to clarify the agreed terms and avoid any disputes in the future.

This Agreement can only be used for providing childcare services. For general and other services, a template of Services Agreement is available on our website.

How to use this document?

The Agreement includes the details of the client (parent(s)/guardian) and the provider, and the details of the children to whom this Agreement is applicable.

This document includes details regarding:

  • details of services and meals provided;
  • location of service;
  • schedule of the service;
  • deposit and fee details;
  • responsibilities of each party;
  • privacy of the child;
  • permissions given to the child in relation to different health-related matters, special health conditions and support required for the children;
  • the details of medications followed by the children;
  • the details about tetanus/booster shots of the children;
  • the details about preferred medical practitioners, health insurance details, emergency contact details, etc.

Once the document is filed, it can be printed on a non-judicial stamp paper of value prescribed by the concerned state and can also be notarized. A copy can be given to the Provider and if required a copy can also be provided to the regular health care provider. A copy also needs to be kept with the client (parent(s)/guardian) for any future reference.
