A Physician Services Agreement is a document used by two parties when a medical facility or practice, also known as a healthcare center, would like to hire an independent healthcare provider to render medical services to patients that they are referred by the healthcare center. This Agreement allows the physician and the healthcare center to outline the scope of their relationship and describe the specific medical services that will be provided, as well as any of the physician's other responsibilities while working for the healthcare center.

This document should be used for healthcare centers that wish to bring on a physician to work for them as a contractor without hiring them as a full-time employee. This is common in situations where a healthcare center needs a particular sort of medical specialist who will provide very specific services to patients or consultations to other doctors at the practice. This sort of Agreement is also common when a physician has their own private practice but also works at a healthcare center on the side. Applicable law

All Service Agreements are subject to both federal and state laws that cover general contract principles governing how contracts are created and interpreted by courts. Federal laws may restrict what services can be contracted for (for example, illegal services may not be contracted for) and certain broad categories, like contracting for something that looks more like an employment relationship, but individual state laws may govern the interpretation of the contract in case of a dispute. There are also individual state laws controlling what a physician must do to receive and maintain their license to practice medicine, such as passing specific board examinations or completing continuing education courses.
