This document is a letter that can be used to cancel a subscription (such as a magazine or insurance policy) or a membership (such as a gym or club). By sending a letter, the member or subscriber can avoid any misunderstanding that may arise from the cancellation and have written proof of the request for cancellation.
How to use this document
It is important to first read the subscription or membership contract that is being cancelled. There may be provisions that relate to the manner of canceling, such as if any forms are needed or the time period or other conditions that must be met.
Upon reading and the subscription and noting any important provisions, complete the required information in this letter. Once completed, the letter should be printed and signed in at least two (2) copies.
If there is a required payment of a cancellation fee, the payment may be enclosed in the letter.
The member/subscriber may send the letter by registered mail to have proof of receipt. The member/subscriber may also personally deliver the letter to the company/business from which the account will be cancelled. In this case, the member/subscriber should ask the person receiving the letter to sign and write down the date of receipt on the second copy of the letter as proof of receipt. The member/subscriber should keep the second copy with the proof of receipt with them.
Any attachments to the letter, such as a copy of the contract or any receipts, should be copies of the original. The original should be kept by the member/subscriber.

In addition the law on contracts and obligations under the Civil Code of the Philippines may also be applicable.
